How do I get a referral to a specialist?
If you need assistance from CFMC for a referral, we would be happy to refer you to a specialist or recommend one for you.
If you have questions about the status of a referral or need it to be renewed, call the CFMC location that you visit.
How can I get a copy of my medical records?
An Authorization to Disclose Protected Health Information form will need to be completed. These may be obtained at the center you usually visit or by calling Medical Records at 270-858-6655 ext. 2341.
- You will be asked to present your photo ID upon requesting and picking up your record.
- The Medical Records Representative will contact you once your records are ready for pick up.
- The Center will not allow anyone other than the patient or legal representative on file (parent, legal guardian, etc.) to pick up the records.
How long will it take to get my records?
Requests are processed in the order they are received. Please allow 7-10 business days from the date that the signed request is received for records to be mailed. If you need your records immediately or you prefer to pick them up in person, please call 270-858-6655 ext.2341. Please remember that photo identification is required when picking up medical records.
Will I be charged for my records?
The KY state law allows the patient to receive one free copy of their medical record. If additional copies are requested, you will be charged a reasonable fee for the copies. Payment is required before copies can be received.
Can I mail or fax my proof of income, address, or insurance?
No. The patient is required to sign the statement verifying the income, family size, and insurance information provided is accurate and complete after updating their account. Only the patient, parent, guardian, or legal representative may sign this statement.
What documents do I need to provide to be eligible for the Sliding Fee Scale?
For patients who do not have private health insurance coverage or who may be unable to pay the full fee for services rendered at any of our CFMC, Inc. clinics, we offer a Sliding Fee Discount.
Click here to view our sliding fee scale. It is the responsibility of the patient to provide the evidence of eligibility to participate in this program. To do so, patients should bring at least one of the following to the health center:
- Current year’s 1040 tax form
- A pay check stub
- An unemployment stub
- A letter (on letterhead stationery) from your employer stating current salary/wages *
- A letter of reference from a 501(c)(3) non-profit organization on their letterhead may be accepted if none of the above are available.
How do I get a refill for my prescription?
Please call your local pharmacy for a refill. If there are no refills available, the pharmacy will contact your provider. If the prescription cannot be refilled, the pharmacy will advise you to contact your provider to schedule a visit. No narcotics will be refilled by phone. All narcotic refills require an office visit.
Do you need to seek medical advice from one of our providers but our facility is closed?
An on-call nurse service is available when the office is closed. This line can be used for non-emergency clinical advice after hours. Please call the clinic you usually visit and follow the prompts.
What do I do if I am sick when CFMC is closed?
For emergency care, please call 911 or go to the nearest Emergency Department at a local hospital. If you or someone you know is struggling with a mental health emergency or in crisis, help is available. Please call/text 988 or go to your nearest emergency room.

Our immediate care clinic offers treatment for a wide range of non-life-threatening illnesses. A medical provider is always on site, so patients can be sure that their treatment will be professional and with as short a wait time as possible.
Our First Choice Immediate Care locations are a convenient, quick alternative for treatment of common illnesses and injuries. Common illnesses treated include allergies, sinuses, cold, flu, cough, bronchitis, nausea, vomiting, diarrhea, sore throat, and strep throat. Injuries treated include cuts, scrapes, eye and ear injuries, sprains, and strains. Sports physicals, flu shots, and diagnostic lab testing are also offered.

Evidence-Based Care is an approach to medical practice where the provider makes decisions about the patients’ care based on evidence from well-designed, well-conducted, up-to-date research.
